Clinical History
This is a 67-year-old female with relapse of refractory low-grade, non-Hodgkin's lymphoma.
Technique
In the Cancer Center treatment room, the patient was infused with Rituxin (250mg/m2 or 470 mg) over a four hour period. Once the infusion of Rituxin was complete, the Nuclear Medicine Technologist infused 5.0mCi of 111In-Zevalin over a 10 minute period (the infusion included a 0.22 micron filter attached to the end of the syringe).
Findings
On the 3 hour post-injection wholebody scan, there is normal distribution of the 111In-Zevalin in the heart, lungs, liver, spleen and soft tissues. In addition, there is an area of increased activity seen in the mid-abdomen, which correlates to the peri-aortic region of lymphadenopathy seen on the recent CT scan. On the 50 hour wholebody scan, again seen is normal distribution of the 111In-Zevalin in the heart, lungs, liver, spleen, with much less activity seen in the soft tissues. In addition, there continues to be increased activity in the peri-aoritc region demonstrating uptake of the monoclonal antibody in the tumor. The change in distribution from the three hour scan to the 50 hour scan demonstrates that the patient clears the activity adequately and therefore she is a candidate for the 90Y-Zevalin treatment.
Impression
- NORMAL DISTRIBUTION OF 111IN-ZEVALIN WITH ADEQUATE CLEARANCE FROM THE SOFT TISSUES, DEMONSTRATING THE PATIENT'S ELIGIBILITY FOR THE 90Y-ZEVALIN TREATMENT.
- INCREASED ACTIVITY SEEN IN THE PERI-AORTIC REGION DEMONSTRATING UPTAKE OF THE ZEVALIN IN THE TUMOR .
90Y-Zevalin Treatment
Technique and Findings
The Zevalin treatment was explained to the patient and informed, signed consent was obtained. In the Cancer Center treatment room, the patient was infused with Rituxin (250mg/m2 or 470 mg) over a four hour period. Once the infusion of Rituxin was complete, the Nuclear Medicine physician infused 25.0mCi of 90Y-Zevalin over a 10 minute period (the infusion included a 0.22 micron filter attached to the end of the syringe). The patient tolerated the infusion well with no immediate complications. The patient is to follow up with her physician. Impression
