Clinical History
The patient is a 36-year-old female with severe abdominal pain,
pronounced under the right ribs radiating to the back. The patient also has a history of mass
cell disease.
Technique
The patient received an IV
injection of 2 mCi of
Tc99m Choletec which was
followed by sequential
imaging over the upper
abdomen at 30 sec
intervals until 10 minutes
and then at 5 minute
intervals to 90 minutes.
Findings
There is prompt filling of
the gallbladder which is
visualized at approximately
20 minutes. At minutes
after injection of the
radiotracer, the patient
received 0.57 ug of
Cholecystokinin. The
gallbladder ejection
fraction is markedly
decreased measuring 22%.
The normal values are
typically greater than 50%.
Visualization of the small
bowel is identified at 50
minutes.
Impressions
DECREASED
GALLBLADDER
EJECTION FRACTION
OF 22%. THERE IS NO
EVIDENCE OF ACUTE
CHOLECYSTITIS,
HOWEVER, THIS
PATTERN MAY
REFLECT CHRONIC
CHOLECYSTITIS.
