Clinical History:
This is a 78-year-old male with metastatic prostate cancer. This study is to evaluate extent of osseous metastatic disease.
Technique
The patient was administered an IV dose of 22 millicuries of 99m Tc MDP, and whole body anterior and posterior planar images were obtained.
Comparison is made to the prior study from 7/30/96. The prior study from 9/18/96 is not in the patient's jacket and is not available for comparison.
Findings
Since the prior study, there has been extensive progression of metastatic disease with diffuse involvement of the patient's axial and appendicular skeleton. There are multiple lesions involving the patient's skull, mandible, entire spine and pelvis. There is diffuse involvement of bilateral lower extremities from the proximal femur to the metatarsal bones bilaterally. There is diffuse involvement of bilateral upper extremities from the proximal humerus to the carpal bones. There are multiple lesions diffusely throughout the ribs bilaterally. There is uptake of the radiolabeled phosphate in the kidneys bilaterally with excretion into the urinary bladder.
Impressions
MARKED PROGRESSION OF METASTATIC DISEASE INVOLVING THE ENTIRE AXIAL AND APPENDICULAR SKELETON AS DESCRIBED ABOVE. IF CLINICALLY INDICATED, THE PATIENT WOULD BE A CANDIDATE FOR STRONTIUM THERAPY.
