Clinical History

This is a 73-year-old patient with left adrenal mass seen on MR, and episodic hypertension by clinical history.

Technique

The patient was administered 1 mCi of I-131 MIBG on 4/12/99 and multiple static images over the head and neck, chest and abdomen, and pelvis were obtained 24 and 48 hours post injection of the radiotracer.

Findings

There is normal uptake of activity noted by the parotid gland, and by the heart. There is normal liver uptake identified. There is focal intense uptake identified in the region of the left adrenal gland, seen well on both the anterior and posterior abdominal images. On the further delayed 48-hour images, there has been a further decrease in background activity, with the focal left adrenal uptake remaining very pronounced. This uptake correlates with the mass seen on the MR study.

Impressions

FOCAL INTENSE UNILATERAL MIBG UPTAKE IN THE LEFT ADRENAL, CONSISTENT WITH AN ADRENAL MASS SUCH AS A PHEOCHROMOCYTOMA.
