Clinical History
A 38-year-old male with deep venous thrombosis. Evaluation for pulmonary embolism.
Technique
The patient received a single breath of 21 millicuries of Xenon-133 gas. Inhalation, equilibrium
and washout images were obtained. The patient was then administered an intravenous dose of 5 millicuries
of Technetium-99m MAA and static and SPECT images were obtained of the lungs in multiple projections.
Findings
Ventilation views demonstrate prompt symmetric flow to the bilateral lungs.The perfusion scan demonstrated
large mismatched segmental perfusion defects involving the anterior basal and laterobasal aspect of the left lower lobe. Large
mismatched perfusion defects are also noted about the anterior and posterior aspect of the right upper lobes.
These findings place the patient in high probability for pulmonary embolism.
