Clinical History
Patient is a 38-year-old male with swelling in the right lower extremity
and prior DVT.
Technique
Gray scale sonographic images were obtained through the right lower extremity
with venous color Doppler evaluation.
Findings
Subcutaneous edema is noted over the right lower extremity. The IVC
appears patent, as does the right iliac vein. The right common femoral vein in its proximal portion appears patent. At the level of the bifurcation, the right
common femoral vein is filled with heterogeneous echogenic thrombus. There is no pulsatility and no augmentation within the common femoral vein. The right femoral vein is duplicated. One of the femoral veins is patent, and the other shows complete occlusion with no flow. The popliteal is partially occluded with thrombus with color images showing a small amount of flow. The left deep femoral vein is
also involved with a clot in the visualized portions of the proximal vein. The greater saphenous vein is patent and compressible.
Impressions
1. DUPLICATION OF THE RIGHT SUPERFICIAL FEMORAL VEIN WITH THROMBUS EXTENDING FROM THE DISTAL ASPECT OF THE RIGHT COMMON FEMORAL VEIN THROUGH ONE OF THE SUPERFICIAL
FEMORAL VEINS TO THE POPLITEAL, AS DESCRIBED.
2. SUBCUTANEOUS EDEMA.
Comments
The color flow doppler ultrasound in January 1999 was normal.
