At the University of Kansas Medical Center, we are currently engaged in a research project using 11C-Acetate PET imaging to detect Prostate cancer. Here are some examples of our 11C-Acetate PET scans.



Clinical History

The patient is a 41-year-old male with prostatic carcinoma.

Technique

The patient was injected intravenously with 42.7mCi of Carbon-11 Acetate. Subsequently, PET/CT images were acquired in coronal, sagittal and axial planes. Imaging was acquired from the base of the brain to the mid thighs. Low resolution noncontrast CT images were obtained for attenuation correction purposes and anatomical localization.

Findings

There is a cluster of at least four lymph nodes near the sacral prominence.

The largest of which is measuring 0.92cm x 1.27cm with increased metabolic activity with SUV measuring 5.45 at table indices #703.

The next largest lymph node is measuring 1.02cm x 0.93cm slightly near to the sacrum with SUV measuring 6.10 table indices #706.

The next largest metabolically active lymph node in the cluster measures 0.53cm x 0.71cm with one additional tiny adjacent metabolically active lymph node noted.

At the distal pre sacrum left of midline there is increased metabolic activity within an enlarged lymph node which measures 1.49cm x 2.09cm with SUV measuring 11.15 at table indices #722.

There is an additional presacral lymph node with metabolic activity at the sacral coccygeal junction which measures 1.19cm x 0.98cm with SUV measuring 4.56 table indices #725.

Metabolically active enlarged lymph node is noted at the right ilial psoas region with SUV measuring 4.29 table indices #703.

Immediately adjacent to the right sacral ala there is increased metabolic activity within a lymph node which measures 0.41cm x 0.83cm with SUV measuring 3.80 at table indices #696.

Adjacent to the left S1 vertebral body and sacral ala there is increased metabolic activity within a lymph node measuring 4.17 at table indices #686.

Anterior to the body of S1 right of midline there is a small lymph node with increased metabolic activity with SUV measuring 4.18 at table indices #686.

Within the spine at T7 there is a focus of increased metabolic activity within the left pedicle with SUV measuring 3.72 at table indices #336.

The bilateral pedicles of T9 demonstrate increased metabolic activity measuring 3.29 on the left and 3.32 on the right table indices #372.

Periaortic/retroperitoneal lymphadenopathy begins at the level of the inferior margin of the liver and stable indices #575 with lymph node measuring 0.92 cm with SUV measuring 7.94.

Two other representative lymph nodes with SUV's measuring 7.48 and 5.95. Each of these lymph nodes measures 0.8cm respectively.

Left periaortic lymph node demonstrates increased metabolic activity which measures 1.75cm x 1.5cm with SUV measuring 10.95 at table indices #627.

Anterior to the left iliac artery distal to the aortic bifurcation increased metabolic activity is noted within lymph nodes with SUV measuring 9.64 at table #663 and measuring 1.4cm x 1.87cm.

There is normal physiologic uptake of C-11 tracer seen within the liver, pancreas and small bowels. Note is made of uptake in the right antecubital vein where the C-11 acetate was injected, which represents a slight amount of extravasation.

Impressions

1. MULTIPLE LYMPH NODES EXTENDING FROM THE SACRAL COCCYGEAL JUNCTION TO THE LEVEL OF THE INFERIOR MARGIN OF THE LIVER IN A RETRO PERITONEAL DISTRIBUTION. THIS IS SUGGESTIVE OF PROSTATIC CARCINOMA METASTASIS. OF NOTE, NO METASTASIS WERE IDENTIFIED ABOVE THE LEVEL OF THE PANCREAS.
2. TWO AREAS ARE SUSPICIOUS FOR METASTASIS WITHIN T7 AND T9. RECOMMEND FOLLOWUP BONE SCAN WITH SPECT.
